Extended Care Services:

San Bernardino Medical Group Extended Care provides convenient medical care for illnesses requiring immediate care, when your primary care physician is not available or when care is needed after regular business hours.  Our Extended Care is open Monday-Friday 9:00am-9:00pm, Saturday and Sunday 9:00am-5:00pm (excluding Thanksgiving and Christmas).

Choosing Your Doctor:

Your primary care physician will oversee all of your health care including preventative care, physical exams and specialty referrals. You will need to call your health plan to select a San Bernardino Medical Group physician.  Usually you must request a physician transfer by the middle of the month for the change to be effective with your insurance by the first of the following month. Family Practice and Internal Medicine physicians are available as primary care physicians.  Learn more about our physicians at San Bernardino Medical Group here. Insurances accepted by San Bernardino Medical Group Once your insurance becomes effective with San Bernardino Medical Group, you will need to call and schedule an appointment to establish with your new primary care physician.  The appointment lines are:

You may also call the Patient Relations Department for more information about physicians or to arrange a transfer from one physician to another within the group.

Medical Records:

If you are a new patient, you will need to complete a medical records release form to have your records transferred from your previous physician.  Please complete the form as soon as you join San Bernardino Medical Group so your physician has your complete medical history.  You may download your Medical Records Release form, complete it and fax or mail it to your previous physician’s office.

Prescription Refills:

San Bernardino Medical Group is an e-Prescriber!  It is an electronic method of generating prescriptions through an automated data-entry process that uses e-prescribing software and a transmission network which links to participating pharmacies.  E-prescribing provides protection, reduces potential fraud and tampering, saves money, is convenient for patients and saves time. Contact your pharmacy with all of your refill requests.  The pharmacy will send your refill requests directly to your physician, cutting out the need for office staff and pharmacy staff to be involved.  If your request is denied, your doctor or nurse will notify you.  Please allow 2-3 working days for the refill procedure.  Do not wait until you are completely out of medication to request a refill.  Some medication requests will require a prior authorization from your insurance company or may require a visit to your doctor’s office.  For mail order prescription programs, please follow the instructions provided by the insurance carrier.
