The Patient Portal allows you, as the patient, to see your pending appointments, lab tests, radiology tests and a whole lot more. It requires both your account name (usually your first initial of your first name followed by your last name) and a PIN (personal id number). If you have never used this patient portal before click on the “Request PIN” option to have your account name and PIN sent to your email address, which we must already have on file for your account. If we do NOT have your email address already on file you will need to to give us your email address in person at your next visit to the clinic. If you already have your account number and PIN just click on “Login” to login to your account.
