The Medical Provider Portal is used for providers with an established relationship with SBMG. Once you have created an account with us you can view claims, referrals, and patient information on patients we have referred to you. To setup an account please email the SBMG IT Liaison at [email protected]. Please leave your Name, Call back number and the providers office information.

San Bernardino Medical Group is pleased to announce that effective September 1, 2019 we will be joining PrimeCare of San Bernardino. We would like to take this opportunity to thank all of our Specialty Providers for the exceptional medical care that has been provided to San Bernardino Medical Group patients over the years.

As of August 30, 2019 the provider portal will be shut down and all referral authorization requests must be submitted via the Primecare Provider Portal. Please follow the same process that you have in place for submitting referral authorizations to Primecare of San Bernardino. Should you have any questions regarding this transition please reach out to your Primecare of San Bernardino Provider Services Representative.
